Supportive Services :

All participants enrolled in LSO’s housing programs, regardless of the specific model they are engaged in, have access to a comprehensive array of supportive services and resources aimed at addressing their holistic needs and fostering their long-term success. These services are designed to empower individuals, an families on their journey towards stability, independence, and self-sufficiency.Within LSO’s housing programs, participants have access to a wide range of supportive programs and activities, including therapeutic art programs, group therapy sessions, vocational training, technical skills development, video production courses, financial literacy workshops, workforce development initiatives, business classes, and more. These programs are tailored to meet the diverse interests and needs of participants, providing opportunities for personal growth, skill-building, and self-expression.

Rapid rehousing Program:

LSO's Rapid Rehousing program is designed to swiftly provide housing solutions for individuals and families experiencing homelessness. Modeled similarly to our Transitional Housing program but with even fewer barriers, RRH offers a low-barrier approach to admission, ensuring that everyone can access housing promptly. Upon entry, participants are paired with a dedicated case manager who collaboratively develops a personalized treatment plan outlining specific goals for stabilization. Typically, participants aim to achieve stabilization within a timeframe of three months. The stabilization phase of RRH focuses on essential aspects such as obtaining identification, securing a stable income through employment or benefits, establishing medical and mental health connections, arranging for sobriety treatments, and outlining pathways for self-fulfillment. Throughout this process, the case manager provides comprehensive support and guidance. Once stabilized, participants are empowered to transition into permanent housing of their choice with ongoing support from their case manager. Alternatively, they may opt to transition into LSO's Transitional or Permanent Supportive Housing programs based on their individual needs and preferences. The RRH program ensures a swift and supportive pathway to housing stability for veterans, fostering independence and long-term success.

Transitional Housing Program: LSO's Transitional Housing program offers a structured environment for individuals and families experiencing homelessness, aging out,re-entry or safe haven,providing a stepping stone towards permanent housing solutions. Participants in this program receive comprehensive support services aimed at addressing their immediate needs and facilitating their transition to stable housing.Upon entry into Transitional Housing, participants are paired with a dedicated case manager who collaborates with them to develop a tailored treatment plan. This plan outlines specific goals for stabilization, typically within a timeframe of six months. Stabilization within the Transitional Housing program encompasses various aspects, including obtaining identification, securing stable income sources, accessing medical and mental health services, arranging for sobriety treatments, and outlining pathways for personal growth and fulfillment. Throughout this process, participants receive ongoing support and guidance from their case manager. Once stabilized, participants are assisted in transitioning into permanent housing, either through independent means or with support from LSO's Permanent Supportive Housing program. The Transitional Housing program provides a supportive and structured environment for veterans, transitional teens,re-entry,homeless,and abused individuals to rebuild their lives and regain stability on their journey to permanent housing.

Permanent Supportive Housing Program(PSH)
LSO's Permanent Supportive Housing program offers long-term housing solutions combined with comprehensive support services for veterans experiencing homelessness, re-entry, domestic violence or transitioning .This program is designed to provide stable and sustainable housing options tailored to the unique needs of each participant. Participants in Permanent Supportive Housing are provided with permanent housing placements for 30% of their total income, typically in the form of apartments or single-room occupancy units. Alongside housing, participants receive ongoing support services delivered by a multidisciplinary team, including case managers, counselors, and healthcare professionals. Support services within the Permanent Supportive Housing program are customized to address the specific needs of each participant, which may include medical and mental health care, substance abuse treatment, life skills training, employment assistance, and social integration support. The primary goal of Permanent Supportive Housing is to ensure housing stability while promoting the overall well-being and self-sufficiency of participants. By providing long-term housing combined with wraparound support services,LSO's Permanent Supportive Housing program offers a holistic approach,fostering independence and improving quality of life.

Home ownership LSO's Homeownership Program offers qualifying individuals a pathway to sustainable homeownership by providing comprehensive support and resources throughout the homebuying process. This program is designed to empower participants to achieve their dream of owning a home while ensuring they have the necessary tools and guidance to succeed. Participants in the Homeownership Program receive assistance with various aspects of the homebuying process, including financial planning, credit repair, mortgage assistance, and homebuyer education. Additionally, veterans may be eligible for down payment assistance and other financial incentives to facilitate their transition to homeownership. The Homeownership Program emphasizes financial literacy and responsible homeownership practices, equipping them with the knowledge and skills needed to maintain their homes and build equity over time. By promoting sustainable homeownership, LSO's Homeownership Program contributes to long-term housing stability and economic empowerment within the community.
